Trinity Health At Home’s team of Licensed Social Workers provide in-home support for caregivers and patients alike. Our social workers offer the proper education for caregivers to best provide for a patient receiving home care. We also provide counseling at home for patients and caregivers as they adjust to their home health treatment.
